You will recall that I handed out a Club Health Check form at the end of May for completion by each member of the Club and to date I have only received 16 forms back.

In order to have a meaningful committee meeting on the 13th it would be vey much appreciated if you could complete the Health Check form and bring it with you to the meeting this Friday. I will have some blank forms if you have mislaid the original.
